What are Fidelity Life's ratings?
Fidelity Life Association is rate A- (Excellent) by A.M. Best. For current ratings go to www.ambest.com

Are these products portable?
Yes, all Fidelity Life worksite products are portable.

Is spouse and dependent coverage available?
Yes, coverage is available for spouses and dependents as well.

Are these products group products or individual products?
LifeTime Benefit Term and Graded Death Benefit Whole Life are group products. Worksite Rapid Decision Term is an individual product.

How many employees or members are necessary to get an offer for LifeTime Benefit Term?
The minimum number of employees or members to obtain an offer is 50. For cases with between 50 and 100 lives, underwriting will be simplified issue only. For cases with more than 100 lives, underwriting is Conditional Guaranteed Issue.

Is guarantee issue underwriting available?
With LifeTime Benefit Term, Conditional Guaranteed Issue is available for groups of more than 100. Guarantee issue may be considered on a case by case basis. Graded Death Benefit coverage is Guaranteed Issue.

For LBT, what are the CGI eligibility (gatekeeper) questions?
Employee:

  1. Employee is actively at work, performing the regular duties of his/her job, in the usual manner and at the usual place of employment.
  2. Applicant has not missed more than 5 consecutive days of work due to illness or injury in the past 3 months.
  3. Applicant has not been hospitalized in the past 6 months.
  4. Within the last 10 years, the applicant has not been diagnosed as having or been treated by a physician for HIV infection, Acquired Immune Deficiency Syndrome (AIDS) or AIDS Related Complex (ARC)?
Spouse:
  1. Applicant has not been hospitalized in the past 6 months.
  2. Applicant has not been seen or treated by a licensed physician or other medical practitioner within the past 6 months.
  3. Within the last 10 years, the applicant has not been diagnosed as having or been treated by a physician for HIV infection, Acquired Immune Deficiency Syndrome (AIDS) or AIDS Related Complex (ARC)?

What do I need to submit to obtain an offer and where do I send it?
You must provide us census information with gender, date of birth and date of hire and a request for offer form (form number MS-1).

When do I get my offer?
If all necessary information is provided, from date of receipt an offer will be created within 2 business days. Once the case is accepted, an offer will be created and forwarded to you along with the Group Master Policy Application (form number: varies by state), and a Case & Agent Form (form number: MS-6 ).

How are premiums / rates determined?
Rates/premiums are determined by the male / female mix of each particular group. Actual rates are unisex and smoker distinct.

Where do I get rates for my case?
Rates will be provided by Vision's Market Support Department based on the deduction frequency of the group. If the deduction frequency is known and provided with the offer request, rates will be provided with the offer.

How do I order quantities of brochures and enrollment forms? Contact Vision's Market Support Department at bgagliardi@visfin.com.

If I am using the website to download forms, how do I know which forms to order?
Your Underwriting Offer will include all forms necessary for enrollment (and form numbers) in the Enrollment Forms and Additional Forms Section(s) of the document; these sections will include signature forms and Leave Behind Forms. The Offer will also outline state specific Signature Requirements.

Yes, you should send Case & Agent Information Form MS-6.
