How do I make changes to information on my policy such as address change, name change, beneficiary change, and owner change?
You can make all of these changes by using our Service Request Form or
Ownership Change Request Form.

How do I change the way that I pay my premiums such as: Change of bank account information or Change of credit card information?
You can use our Premium Payment Option Form to make these changes.

How do I collaterally assign my policy?
You must complete the Assignment of Policy as Collateral Security Form to effect this change.

How do I request a loan, partial withdrawal, surrender, or dividend option change on my policy?
You must complete our Loan/Withdrawal Surrender Form.

When do you send my premium billing?
Premium notices are sent 21 days in advance of the premium due date. A reminder notice is mailed 10 days after the premium due date.

I’ve lost my policy. Can you provide me with a certificate of coverage?
Yes, just email service@fidelitylife.com or call us at (800) 369-3990. Please include in your email the policyowner name and a phone number.

How do I submit a claim?
A beneficiary or someone acting on behalf of the beneficiary can submit a claim using our Claim Form.

How do I reinstate a lapsed policy?
Please email service@fidelitylife.com or call us at (800) 369-3990. Please include in your email the policyowner name and a phone number.
